It looks nice. Under "Service Status" where you have the text in red, I would make like a baby blue #006699 or something like that, would look better color coordinated.
As for the Premium Rapidshare accounts, I didn't take the time to do the survey, but do you actually give out a Premium Account or are you scamming?
If you are just doing this to get someone to fill out a survey and in the end they receive nothing you will ultimately be killing all of your potential direct traffic.
