Alertpay is not currently on the list of approved senders. I also believe that they send transfers to US bank via international wire and not ACH.

You would need to check with PayPal to see if they allow withdrawing to a bank account under a different name than the PayPal account.
Yes, Payoneer services are available to residents of Macedonia.

Yes, we recently added Skrill (Moneybookers) to our white list of approved senders for the US Payment Service.

If Moneybookers allows you to add a debit card to your account, you can add your Payoneer card, as it's a debit MasterCard card.
You will not be able to load funds directly to your card, only make payments. If you'd like to withdraw funds from your Moneybookers account to Payoneer, you can do so using our US Payment Service (which provides you with a US account/routing number that can receive funds from select corporations, including Moneybookers).

You're very welcome!
There is no limit to the amount that you can receive using the US Payment Service. You can receive as little as $0.01, or as much as you'd like. (PayPal sometimes makes test loads to the account for verification purposes, which can be as little as $0.01)
