If you run a forum install this http://www.stopforumspam.com/ if a spammer is still able to join and spam your forum find all the info about this fool in your forums admin panel & add it to the stopforumspam list
Had the same problem and banned lots and lots of fake users created by spam bots but finaly i got to understand how these bots operate.
They search the web for some known vulnerability and use it. by Default Phpbb3 registration page i very simple, this has made spam bots easily find and fill in the fields then for the captcha, they use a OCR to get the characters in the captcha.
From there, they log in and browse through your site looking for 'new topic' button from where they get to work and paste their junk
So how to counter this threat
1. add a new mandatory field in the registration
Use Anti-Spam Control Panel and set the 5 posts as for new members and so they will be in Newly Registered Users group, which means topics will be moderated. Works for my forum.
that works if you have few spammers but assuming you have hundreds of spammers, deleting each spam will be a tedious job. Maybe an option is to prevent posting to all forums in the site for new members other than new members area and general discussion area so if its spam you can easily manage it.
