How to remove your old URLs from Google index?
its very simple all you need to do just use 301 redirect with your .htaccess.
301 redirect your old url to new url in your .htaccess file.

Example:

Redirect301/oldurl.htm http://www.site.com/newurl.php

After sometime google removes your old link from its index.

Webmaster tool is best to remove URL indexed in Google but still you have to wait untill URLs get deindexed.
Also, you can deindex by robot.txt. Just make a folder and list all URL to remove and disallow it via robot.txt.

If you want to remove old urls form your site, the best way is to uploan in webmaster tools new sitemap.
If you want to remove old and bad backlinks for your site, use google disavow tool.

Redirection is long process. Best ides used by may of webmaster is the Google Disavow Tool. This tool have the feature to remove the link which is not necessary for our website. You may get this tool from Google webmaster Account.
