How to input the details in java form using prompt input form using netbeans ??

Hi
I am relatively new to JAVA Programming. I want to enter the details of customer by prompting each input. Similar to JOption Pane box. First dialog it shows the name when we enter it and click next another prompt address then age ,country etc. All the details entered have to be stored into database later. How can I achieve this using Net beans Java Application. Reply ASAP ......
