how to create an online database?

My boss asked me to do an online database where people can look up their application status on internet. At the moment i looked at caspio bridge but they charge monthly subscription at a very steep price.

If i want to create a simple online search database,,,where should i start?
Something as simple as that

You can keep the interface to the basic .., it isn't hard to code a php-mysql script to lookup the status.

All you need is to setup the database and enter the data into it, then use php script to match the unique key (application no. etc.) and return the status.
