Need advice - Making a DMCA Disclaimer Page on my blog

I've been looking around at some sites, especially those which are similar to mine. I'm planning to write a DMCA disclaimer page on my own blog, and I wonder what essential elements should be written there. As of now what I wanted my page to represent:

- That no potentially illegal/copyright infringing sites are being hosted on my site
- That the site is to comply on issues regarding DMCA by contacting the admin
- Contact form for further questions/issues

I wonder if there should be some more to be added/modified.

You don't need much more than that you got the point across its more to protect your own skin , most users don't care to read it m8, so don't knock yourself out. id just add it to the footer widget.
