Help in my Class Assignment! PHP

Hi Guys,

Can anyone help me to create this one? This is my first assignment in school.

Instruction:
Build a simple web application that stores the Name, Phone, Address, Email, and Status. Your application must able to Create, Read, Update and Delete records using AJAX (Jquery). Create a read me file (README.txt ) for your web application. Follow the structure below:

actually i dont know php yet. but our professor give me this assignment.

note: maybe i will give $2 for the one who can give the code for this one. i really need to pass this.
