Best Web Development Program, where do you start?

So over the past decade in-between graphic design i have learned alot of basic coding techniques, i can comfortably edit a web document and/or forums and wordpress without much of an issue as i understand terms and how things work.


That being said i've never physically designed my own website from scratch, my question today and would like to have a discussion over this is the following;


1- Is there a software in which designing websites from the ground up is more ingenuity? I own Notepad ++ & Dreamweaver, love both of these especially notepad ++.

2- Whenever you design a webpage do you start from a blank document, or is there a readily available template that has all your basics typed out for you? being the likes of <!DOCTYPE html> etc.


How do you all work, what program do you like to design is or 'programs' perhaps you work between multiple software.
Do you always start with a blank page or do you like using rendered templates so you can fill in and then customise it yourself? :)
