You need to recompile kernel to include NAT. Once this is enabled and server is rebooted, make sure the following entry is there in the lsmod output.

You will need to check the full headers of the spam mail. It will contain some sort of clue. It can be that your mail server IP is listed in the spam database IP listing. In some instance the lack of reverse dns for the mail server IP will cause the spam issue.
You can configure the server by adding a daemon or something that will be constantly running and changes the main server IP at your desire. By this way you can make a server with dynamic IP. But the IP ranges should be provisioned to your server by the datacenter.
